2. Have ready: your name and date of birth, your current pharmacy name and phone number, and the medication(s) you want moved.
3. Our pharmacist will verify your information and contact your pharmacy to start the transfer.
4. We will call or text you when your prescription is ready for pickup or delivery.

• This form sends a request only. It does not guarantee a transfer.
• Controlled substances (Schedule II–V) may not be transferable depending on Florida law. Our pharmacist will advise you.
• A licensed pharmacist will verify your information and contact your current pharmacy directly.
• By submitting, you authorize A&A Family Pharmacy to contact your current pharmacy to request transfer of the prescription(s) listed.
